Official Transcripts, and Records

Please follow the instructions below

TRANSCRIPTS AND RECORDS MUST BE ORDERED VIA REGULAR POSTAL MAIL (see details below)

Educational Institutions, Parents, and Adult Former Students of Keystone may request copies of TRANSCRIPTS, OFFICIAL TRANSCRIPTS, and RECORDS for a FEE of $5:

• Requests should be mailed to:
  2000 N. Wells St, Bldg. 2, Fort Wayne, IN 46808
• All requests must include $5 fee payment, in the form of a check made out to Keystone Schools. Do not send cash.

PLEASE NOTE THE FOLLOWING DETAILS AND GUIDELINES:

GENERAL

1. You must provide the last year of attendance to ensure that the correct records/transcripts can be located
2. You must provide your maiden name (if applicable) to ensure that the correct records/transcripts can be located
3. You must provide accurate and complete addresses or fax information in order for records/transcripts to mailed or faxed
4. Fees must be received before requests will be processed

TRANSCRIPTS

1. Transcripts are available only for high school students and those middle school students who completed any high school courses for high school credit
2. Transcripts can be mailed to you or directly to any institution you indicate, as long as addresses are provided
3. Transcripts do NOT include SAT or ACT scores. You must request copies of your student's SAT or ACT scores directly from the College Board

OFFICIAL TRANSCRIPTS

1. Official Transcripts can ONLY be mailed directly to the institution requesting or in need of the official transcript
2. Official Transcripts include an official school signature and embossed marking as proof of validity

RECORDS

Please note that "complete records" include the following from the hard files:

1. Medical records
2. Test results (NWEA, ISTEP, PSAT, SAT, ACT only when available since not all students took or participated in all of the tests)
3. Last issued report card for students who attended the 2009-2010 school year ***
4. Copy of transcript for students in grades 9-12
5. Copy of transcript for student in grades 6-8 only for those who completed any high school courses for high school credits

*** Records do not necessarily include copies of report cards for any year prior to the 2009-2010 school year.
